Information for New Clients

Please complete the necessary forms below and bring them with you to your first visit. Your completed forms will help us understand your specific needs and will be reviewed with you by your therapist. Based on your specific therapeutic needs, you will be advised on which technique(s) are best to support your healing.

Rates and Insurance

Sessions are available by appointment only and take place at our office in Franklin, TN or Reston, VA. Telehealth sessions are also available. Cost per session ranges from $135-$185. Pay by Cash, Check, American Express, Mastercard, Visa.

 

I do not participate in network with insurance programs.

A Superbill is available upon request for you to seek out of network benefits reimbursement.
